The intent of outreach is to identify elders with unmet needs and to increase awareness and understanding of the support services available to them and their caregivers.
The outreach program identifies the needs of all residents 60 and over. Once the need is identified, the outreach coordinator provides information and referral services and links the appropriate agency to the needs of the elder. In this fashion, the program is able to connect residents to agencies and services with the eldercare network and to make up-to-date information and referrals available to residents.
All information that is gathered is held in strict confidence.
