M.G.L. Chapter 90, Section 26 requires a person who was operating a motor vehicle involved to complete the Motor Vehicle Crash Operator Report form (PDF) in a crash in which:
- Any person was killed
- Any person was injured
- There was damage in excess of $1,000 to any one vehicle or other property
Report with the Registrar within 5 days after such crash (unless the person is physically incapable of doing so due to incapacity).
You should not need to file a crash report if the crash occurred on a private road, driveway, parking lot or other private ways.
Steps to Follow After Being Involved in a Motor Vehicle Crash
- Download and fill out the RMV Motor Vehicle Crash Report Form.
- Operator must send one copy of the crash report to the Blackstone Police Department, one copy to your insurance company and one copy to the RMV at the following address: Registry of Motor Vehicles Crash Records P.O. Box 55889 Boston, MA 02205-5889.
- To obtain a copy of the Police Report, please visit CRASHDOCS.ORG and provide your accident number (example of an Accident number: 23-18-AC ).
Police reports are generally available on CRASHDOCS.org within 5 to 7 business days (Police Reports are free).
We will only release a copy of the Officer's report after the department has received the operators Motor Vehicle Crash Report Form.
